текущая версия имеет проблемы в текущем окружении:
1. TYPE=MyISAM не срабатывает. Я просто удалил укзание типа и всё заработало.
2. http://www.maxblogs.ru/articles/oshibka-v-rabote-adminpaneli-sams
3. Желательно отвязать зависимость на MySQL-client, заменив на /usr/bin/mysql , чтобы можно было использовать альтернативный mariadb-client
