В связи с тем, что в scalable лежат PNG 128x128 пикселов, в Qt-шных приложениях они отрисовываются в этом размере. Преобразование в SVG в том же каталоге решает проблему.

Достаточно удалить каталог scalable и его упоминание из index.theme .
